03	Ethics Review Certification Ethics Review Guide 2013
04	A signed Photo Usage form if you are submitting an updated photo (which is different from what is currently posted on the DBB website).

OTHER DOCUMENTS, IF CHANGED OR RENEWED:
01	A current copy of your bio, if changes have been made since you last submitted a bio.
02	A color copy of the signature and picture pages of your US Passport, only if it has been renewed.
